Vision Insurance is a health and wellness plan designed to lower the cost of routine preventative eye care and help pay for prescription eyewear or contact lenses. Some vision insurance plans even asset with corrective surgery. Vision insurances is often obtained through your company, association, school district, or a government program like Medicare. What must I bring with me to my appointment?

We require you to bring your insurance cards, and a valid ID card to each appointment.

What is covered by my vision insurance plan?

Most plans will cover a yearly routine eye exam, some will cover every 2 years. Some plans will give a yearly allowance to cover materials such as frame and lenses, or contact lenses. The amount depends on individual plan you have signed up for. We will pull your benefits and inform you of your coverage amounts at the time of your appointment.

What is covered by my medical plan?

Your medical insurance covers any office visit that is not considered routine vision, whether it be for testing or any medical issues that you are seen for. Please note you will be responsible for any part of your balance towards your deductibles or coinsurance.

What payment will I be responsible for on the day of my appointment?

You will be required to pay any outstanding balances, copays, and if you purchase glasses or contact lenses any balance that is not covered by your insurance.

What happens if I forget about my appointment?

If you do not notify us that you are unable to keep your appointment with a 24 hour notice, you may be charged a $25.00 no show fee.

What type of payment options does Affinity Eye Care offer?

We accept cash, checks, Visa, Mastercard, American Express, Discover & Care Credit.

What is Care Credit?

Care Credit is a line of credit you can apply for to pay for services and materials. This line of credit has a deferred interest rate if paid in full during the promotional period.
